Background: Hywel Dda UHB and Welsh health-tech start-up Haelu are working towards more preventative and efficient community care, by helping Care at Home teams become more digital in their care of reablement and palliative patients.
Pembrokeshire’s Care at Home team were identified through interviews to be an ideal test bed for this project, given the important work they do together and links to Coordination Centre, District Nursing, Therapies and the Acute Response Teams.
The Project: To digitise the current documentation and process for raising concerns for the Care at Home team. We will develop a mobile application for staff visiting patients, to collect information from visits, using the format of existing processes e.g. Fundamentals of Care and Record of Care. The app will link to a dashboard that enables managers to remotely access these visit logs and over time begin to evaluate trends and areas for improvement.
Approach: Haelu handle product development and have worked with the Care at Home team to co-design the app. The pilot is scheduled for July 2023, lasting for 3 months, with an evaluation supported by Hywel Dda UHB, following this we will then scale to other Hywel Dda regions.
Anticipated Benefits:
- Increasing time for direct care and providing more person-centred care, through improved access to data by Healthcare Support Workers
- More quickly identify and communicate patient needs using digital notes and prompts for Management teams at Care at Home or Coordination Centre
- Remote access to patient information to manage risk while pending referral and monitor for improvements post intervention
- Provide coordinators with data/information needed to triage to the right person at the right time and better notes for those receiving referrals
- More proactive care that reduces resources spent on emergency services
