The West Wales Regional Partnership Board Conference and Awards Ceremony 2025 will showcase the work undertaken by the health and social care sector across West Wales. Highlighting the latest innovations in health and social care in West Wales, learning from partners across the region.
The Conference will follow the theme of ‘Integrated Community Care’, highlighting the importance of care in the community. The event will focus on showcasing the latest innovations and best practices in health and social care, as well as celebrating the achievements of individuals and organisations that are working to improve the health and well-being of people in Wales.

Tech Enabled Care in West Wales
Technology Enabled Care (TEC), is the use of technology to provide health and care services to people in their own homes, or near to home, helping people to live independently and safely. Staff from across West Wales will be demonstrating what is available and how this has benefited the community.

Capital
The development of Integrated Health and Social Care Hubs remains a policy commitment for the Welsh Government, supported by the RPB Capital Programme. However, challenges exist on the availability of funding and ability to progress existing schemes in the pipeline. Learn how a recent case study from the Carmarthenshire Living Well Centre is giving a different perspective on the “blueprint” for future integrated hub type projects. There will be opportunity to engage in debate on the future “blueprint” for these projects in the revised regional capital plan.

Workforce
The Workforce workshop will provide a background to the role of the Regional Workforce Board (RWB) and the recent progress made by the four workstreams developed by the RWB to meet the region’s needs.
The workshop will take a deep dive into the issues surrounding the recruitment and retention of young people into the Health and Social Care sector. This will include exploring the possibilities available to expose school aged children to Health & Social Care roles to aid them in making informed decisions about their future careers. As well exploring a clear pathway to work based placements in an equitable and affordable way. We welcome you to attend and share your ideas in shaping workforce planning for West Wales.

Carers and Dementia: Workshop Part1
This workshop will be exploring the priorities for the West Wales Carers Strategy 2025-2030 and will provide an important opportunity to help contribute and shape the future of support for unpaid carers in West Wales.
Anna Bird and Naomi McDonagh will be guiding us through this important discussion and will help focus on the most pressing challenges, identify opportunities, and work collaboratively to ensure the West Wales Carers Strategy meets the needs of unpaid carers across the region.
The workshop will be interactive, so we encourage everyone to participate, share ideas, and ask questions as we navigate the key priorities and objectives that respond to carers’ needs over the next five years and beyond.

Carers and Dementia: Workshop part 2
The co-production, design and delivery of A Good Life With Dementia.
We are grateful to Peter Clark and Damian Murphy who will be presenting on the co-production, design and delivery of ‘A Good Life With Dementia’ in West Wales.
‘A Good Life With Dementia’ is a post-diagnostic course created by and for people living with dementia. This presentation will explain the initial steps of developing and delivering the course in West Wales, with input and help from the voices of lived experience and support from Innovations in Dementia.
The course will help those newly diagnosed with dementia by sharing personal experiences and providing important information during a time of uncertainty and will offer a safe and supportive environment for people living with dementia to answer key questions and to learn from each other.

Children and Young People
The purpose of this workshop is to explore Artificial Intelligence (AI) and the implications for children and young people. Artificial Intelligence (AI) is rapidly transforming society, impacting education, healthcare, social services, and the digital environment in which children and young people grow up. Children and young people are increasingly interacting with AI in their daily lives, through the use of smart devices, engaging with online content, or through use of services where AI systems inform decisions about children and their families.
There are a number of factors to consider in terms of understanding both the opportunities and risks of AI and ensuring AI benefits children and young people, while safeguarding their rights and well-being. AI presents specific opportunities and challenges for children and young people, although children and young people are typically underrepresented in decision-making around AI. This workshop aims to explore pro-active and collaborative actions that prioritises children’s rights and well-being, by considering the development of ethical AI policies, digital inclusion and literacy and safeguarding measures, in terms of shaping the impact of AI.

Integrated Care/ Home from Hospital
Hywel Dda University Health Board (HDUHB) has introduced Porth Preseli in Pembrokeshire and expanded the scope of the Urgent and Intermediate Care (UIC) Team. Porth Preseli provides a single point of contact for all referrals, ensuring effective triage, with the most urgent clinical cases being supported by an integrated UIC response which includes WAST, Frailty, SDEC, Social Care, Third Sector, and Out of Hours. This urgent and holistic response has proven successful in reducing emergency hospital admissions and the need for WAST conveyance, with people receiving the support they need to remain at home safely.
This way of integrated working has been a springboard to the Urgent and Emergency Care Clinical Model in the region and our Hospital at Home ambition.
This workshop will provide an overview of the UEC clinical model, current Hospital at Home models in the region and our future vision for what Hospital at Home could provide to our population. As part of the session, we will be facilitating the discussions to gather valuable insight from the attendees to help inform the model going forward.

In my current role I lead a small corporate team who work across the Health Board to co-ordinate actions focused on promoting equality, diversity and inclusion and addressing barriers experienced by minority ethnic or other groups who are vulnerable to experiencing disadvantage.
I currently chair the West Wales Carers Development Group (part of the Regional Partnership Board governance), working with colleagues from across the Hywel Dda area to deliver our regional Carers Strategy and improve the lives and experiences of unpaid carers.

How many tickets are you allowed?
This event is invitation only, but there will be a waitlist for those who are interested in attending. If you think your colleague would benefit from attending, please contact us to apply for a waitlist ticket.

Tea and coffee will be served throughout the morning and afternoon, as well as a complimentary lunch.
Will there be onsite parking?
Parc Y Scarlets has free parking, right outside the stadium. To reach the event, please use the main entrance. Disabled parking is also available.

The dress code for the conference and awards is usual office attire.
