As a region, we’re collaborating to transform care services in West Wales for adults, children, and unpaid carers. Your voice is essential in driving change! Explore some of the real-life experiences shaping our health and social care improvements below.

Our case studies illustrate how health and social care initiatives align with the strategic goals of the Health and Social Care Regional Integration Fund (RIF) and the West Wales Regional Partnership Board (WWRPB) priorities. By showcasing the tangible impact of these efforts, the case studies underscore the progress and value these programs bring to those in need of care and support in our West Wales communities, emphasizing the region’s dedication to integrated care and inter-agency collaboration. 

The focus of our case studies extends to other key priority areas, including programs that tackle specific health challenges and early interventions, support for elderly, disabled, and other vulnerable groups, and projects that promote the sustainability and efficiency of health and social care services. We also highlight initiatives addressing workforce challenges within the sector.

WeCare Wales at Milford Haven School

WeCare Wales visited Milford Haven School to speak with students about careers in social care. Many of the young people—some of whom are young carers—recognised that the skills they use every day are the same ones needed in care roles. This inspiring case study video highlights their reflections and shows how the school actively supports its young carers. 

Reablement in West Wales – A Case Study

Primary Care Project – A Case Study in Early Intervention

This video explores a primary care project in Pembrokeshire that supports people before they reach crisis point. By working closely with individuals who frequently present at GP services, the project offers early occupational therapy interventions focused on self-management, proactive care, and prevention.
